We operate solely on a private pay basis and do not accept any form of insurance. However, we provide a superbill, which parents may submit to their insurance companies for potential reimbursement.

How Can I Schedule An Evaluation?
Please visit our ‘Schedule Now’ page to fill out a brief form and our office manager will contact you within the next business day with all the necessary information to finalize your appointment. You may also call our office directly.
Please complete all required paperwork using the IntakeQ portal link that was sent. We will contact you a few weeks before your appointment to ensure we have the necessary information. Our providers must review school or clinical evaluations conducted within the last 18 months. This professional requirement ensures the data obtained are considered valid. Even if you disagree with the findings of a recent evaluation, our providers will request access to this information prior to the evaluation to ensure tests are not duplicated.

Our providers have considerable experience evaluating children with a broad range of neurodevelopmental differences, genetic and medical conditions, and mood presentations. These include but are not limited to, ADHD, Autism, Asperger’s, Giftedness, Tourette’s, Dyslexia, Dysgraphia, Dyscalculia, Intellectual Impairment, Dyspraxia, Sensory Processing Disorder, Speech-Language Disorders, Depression, Anxiety, OCD, Bipolar Disorder, Traumatic Brain Injury, Autoimmune Disorders (PANS/PANDAS), Fetal AlcoholSyndrome, Cerebral Palsy, Down Syndrome, and Epilepsy.

You can expect your child to work with their provider for a large portion of the evaluation. Our providers also use other qualified professionals (doctoral trainees, psychometricians), a common practice, to help during the testing process. This is intended to obtain important clinical observations and data from multiple sources.
Yes. Our office is wheelchair accessible. When indicated, adjustments to the testing procedures and the environment can be made for people with physical, hearing, or visual impairments.
In Arizona, once a child reaches the age of 18, they are considered an adult. Patients 18 years of age and older must complete their own Policies & Procedures documents and provide their informed consent before the evaluation. Keep in mind that if your child turns 18 prior to their scheduled evaluation date, they will need to complete this paperwork.
Yes. If there is shared custody, all those with decision-making authority must agree to the evaluation and sign all Consent forms. In cases where sole custody or decision-making authority exists, our office requires legal documentation of sole custodial award or decision-making authority. Without legal documentation of sole decision-making or a sole custody award, we cannot proceed with an evaluation.
The Multidisciplinary Evaluation Team (MET) reviews school and outside evaluator data to determine if there is sufficient information to determine if a child is eligible for special education and requires specially designed instruction to access the curriculum. Once a child has qualified for special education, the Individualized Education Program (IEP) outlines specific curriculum modifications and instructional accommodations. A 504 Accommodation Plan outlines accommodations a child may need based on a suspected disability.
Our providers have extensive knowledge of special education law and procedures, and thus our evaluations are designed to align with the various special education eligibility requirements. If your child is eligible for special education, the appropriate eligibility category or categories will be listed in the report. Schools will have all the data needed to determine special education eligibility and information related to their instructional needs will also be included. However, in some cases, school professionals may suggest additional information is needed. When this is the case, we strongly encourage parents to hire a special education advocate to help them navigate this process.
Navigating the school system can be challenging, and given that the educational options in Arizona are constantly changing, we recommend parents consult with an Educational Advocate. While it is not necessary for all families to consult with an advocate, we strongly recommend working with these specialists if there is concern that your child’s school is resistant to providing support.

How Is Payment Handled?
Payment is due in full on the day of the evaluation. We accept Credit, Debit, Cash, Check, ESA Debit Card and ClassWallet (ESA). Once the clinical report has been completed, we will provide you with a superbill for services rendered. For those using ClassWallet, please click here to understand the payment process.
If your child is prescribed medication, have them take it as prescribed on the appointment day. Please ensure your child brings their glasses or other assistive devices (if applicable). We encourage you to bring any snacks or drinks your child prefers, which will be accessible to them throughout testing.
All evaluations begin at 9:00 am on your scheduled appointment date. There is no need to arrive early. Most families can expect to be here for approximately 3 to 4 hours. During this time, your child will undergo various tests by their provider and support staff. You can expect to meet with your child’s provider to review the background paperwork you provided to gather additional information. Following testing, we will schedule a feedback appointment for the following week to review evaluation impressions.
We typically schedule our feedback appointments virtually for convince. This appointment typically takes 60-90 minutes, during which your provider will review testing outcomes, diagnostic impressions, and recommendations. This information will then be compiled into a clinical report, which you expect to receive approximately two weeks after the feedback appointment.
The report will detail all essential information about the evaluation, including background information, testing data, clinical impressions, diagnostic codes, and clinical and educational recommendations.
If you need to modify or cancel your upcoming evaluation, we request you contact us at least two weeks in advance so we can do our best to accommodate you. While we do our best to reschedule appointments within a few weeks of the initial date, we cannot guarantee that we will be able to fulfill specific date requests.
Yes. Arizona Neuropsychology Centers employs a certified Emotional Support Animal (Iggy), who will be in the office on the appointment day. Iggy is a friendly, hypoallergenic, non-shedding dog who is a valuable office member.
Children often have many questions about the purpose of testing and what they will be expected to do. Below are questions and answers we encourage you to discuss with your child before their appointment.
Why Am I Being Evaluated?
Let’s start by stating the obvious. There is nothing wrong with who you are! We often work with kids who struggle with things like paying attention, recalling directions, making friends, or with schoolwork. Our tests help parents and teachers understand how you think and learn, giving them the best tools to support your amazing brain.
You will work with a doctor to complete many different ‘brain games.’ These games can involve solving puzzles, playing memory games, drawing pictures, building with blocks, and completing speeded races. We will also do a little reading, writing, and math – but don’t worry, it won’t be like school!
We typically tell kids they can expect to be here for around 3-4 hours. We know that sounds like a long time, but you will have many breaks to ensure you can relax and feel comfortable. Most kids can’t believe how quickly their time goes here. When you are all done with the brain games, you will have a chance to pick out some prizes for all your hard work.
Nobody knows everything! There will be some brain games that you will find easy, while other tasks might seem a little more challenging. We want you to try your best and have fun!
Great question! We have a special office dog named Iggy, who is typically in the office during evaluations. Iggy is very friendly and enjoys meeting new kids.
Yes. There are multiple scheduled breaks throughout testing, during which you can stretch, use the restroom, hang out with your parents, play with Iggy, etc. We are also happy to provide extra breaks when needed. While we want you to give us your best effort, we also want you to feel comfortable. You can always let a provider know if you need a break.
Yes! We encourage kids to bring their favorite snacks and drinks to enjoy while working. Don’t worry if you forget your snacks or run out; we have a bunch here at the office for you to pick from.
Absolutely not! While there is nothing to be embarrassed about, your friends will not know about the evaluation unless you tell them. What we do during the evaluation is private and only shared with your parents and sometimes school.
Most of the time, you will only need to come for once to complete the brain games. However, sometimes, kids come back after a few years for a check-up to see how they are doing and if there are any new ways we can help.
