***Availability Update***

Dear ANC Families,

Due to an unforeseen family medical situation, I have had to make significant adjustments to my schedule. I deeply regret any inconvenience this may cause families seeking timely evaluations and truly appreciate your understanding. Our Practice Manager, Ms. Suzi, and I are working out my schedule for the next six months, and we appreciate your patience during this time. There may be longer than usual responses to communications and wait times to obtain an evaluation.

To ensure continued support for families, my trusted and experienced colleague, Dr. Volrich, remains available to provide evaluations with the same level of care and professionalism you expect from ANC. Additionally, I am actively working to welcome another skilled and compassionate colleague to our team to help meet the needs of our community.

Thank you for your patience and trust as we navigate these changes. Supporting you and your child remains my utmost priority, and I am grateful for your understanding during this challenging time.

If you are prepared to schedule an appointment, please complete and submitt the inquiry below.
Our Practice Administrator will contact you to be placed on our calendar and provide you with the Developmental History document and Consent Forms to be completed before the appointment day.

STEP 1: Complete and submit the Patient Information.

Step 2: After receiving your email, our practice manager will respond with the next available appointment date. Upon confirming the appointment, you will receive a link to our INTAKEQ portal. To ensure the confidentiality and security of your information, our IntakeQ portal is HIPAA-compliant and privacy-protected.

STEP 3: Through the INTAKEQ portal, you will be able to complete the Developmental History Intake providing information about your child’s development background. Sign the necessary consent forms to allow us to proceed with the evaluation. Upload Required Documents and relevant documents, including: Past psychological evaluations School evaluations (IEPs, METs, 504 plans) Medical records (if applicable).

ADDITIONAL INFORMATION

The developmental history and information provided through the INTAKEQ portal are critical to the evaluation process. The evaluation is based on this information, and any inaccuracies or omissions may affect the final report. Please be thorough and ensure that all relevant information is included. Duplicate tests will invalidate the final report, and we cannot be held responsible for incomplete information.
